Unhappy Problems booting into maintenance mode

Yesterday I had to boot my vio server into maintenance mode to be able to set the padmin password.
(either the users is locked, or someone have changed the pw without my knowledge )

I both tried to allocate a spot (which is the one that the vio server is installed from), and I tried to 'smitty nim_mac_op', and here allocate the same spot (I also tried to allocate another spot at the same aix level)

I booted into the sms menu
Chose to boot from network
Chose 'Select Access this Wolume Group and start a shell'

In both situations, I didn't get the opportunity to choose a disk to boot up to, so I enden in the 'OK' prompt, without access to /usr, so I couldn't set the passwd, becauseI got the message
/usr/lib/boot/getrootfs: no such file or dir.

Does anyone have any idea of what is wrong ????
Please help.....

I couldn't choose it when entering this post, but I'm on a Power6 server, not Power5

Ahhhhhh, Houston - you have a problem....

The only way to do this is to boot the VIO from the install media. A NIM generated VIO spot does not support a maintenance boot.
